Here you can download all versions of SFU. I recommend the latest version (v1.0) of course, but please go ahead and use 0.9.3 as well (they work in parallell). I will continue to support 0.9.3 as there has been many requests for that...

Is there a way to add this (or the 0.9.3 ver.)to the User Menu? I'd like to put it right by "Submit an Article" and "Submit a Link" menu items.
The only way I can come to think of is to add it in an Article (check the FAQ how to do it) and then add a menu for the Article.

i installed mod_simpleFileU pload.0.9.3 but, there's error : Notice: Undefined index: uploadedfile in C:\wamp\www\Joo mla\modules\mod _simplefileu pload\mod_simplefi leu pload.php on line 51.

one issue was that when i refresh the page the form is sent again and again. is it on pageload?
Ya, reload of the page sends the HTTP request for upload again. If you use redirect of the page it should not do it...
